Thinking about hormone therapy while on Zepbound or Wegovy? Here is how to prepare, what to track, and what to ask about the form of HRT.
For 2–4 weeks before your appointment, log:
| Question | Why it matters |
|---|---|
| Pill, patch, gel or spray? | Oral forms go through the stomach. Skin forms don’t |
| Do I need progestogen? | If you have a uterus, estrogen is paired with a progestogen to protect the lining of the womb |
| What about my GLP-1 dose schedule? | Try not to start HRT the same week as a dose increase, so you can tell effects apart |
Give it time, and don’t judge it on the first fortnight. Hormone therapy is the most effective treatment there is for hot flashes, but how quickly it settles varies, and your prescriber will have told you what to expect for your particular regimen. Some bleeding can be expected with certain regimens at first.
Book a follow-up after about 3 months. Bring your log.
Dana, 49, tracked 3 weeks of night sweats before her appointment. She started an estradiol patch two weeks after her last Zepbound increase, so the timing wouldn’t overlap. Her log showed night sweats dropping from 5 nights a week to 1 within a month.
It’s easier to see what’s causing what if you space them out. Ask your prescriber.
Patches don’t depend on the stomach. Whether it’s better for you depends on your health history.
There’s no clear evidence either way. Track both and review with your doctor.
