PMS and cravings on a GLP-1: does your cycle change your appetite?

Hungrier before your period, even on Wegovy or Zepbound? Here is how your cycle can affect appetite, and how tracking helps you plan.

Key points

If your GLP-1 keeps your appetite quiet all month except the week before your period, you’re not imagining it.

Why your cycle affects appetite

After ovulation, progesterone rises. Many women notice more hunger, cravings for carbohydrates or sweets, bloating and mood changes in this phase. ACOG’s list of PMS symptoms includes changes in appetite and bloating, along with mood swings, irritability, anxiety, trouble concentrating and sleep problems. (ACOG says “changes in appetite” rather than “cravings” — the craving for something specific is what women tend to describe, and it fits inside that.)

How it may show up on a GLP-1

Cycle phaseWhat some women notice
Period (days 1–5)Tired, crampy, appetite often steady
Follicular (after period)Appetite well controlled
OvulationVaries
Luteal (week before period)More hunger, cravings, bloating

Your pattern may be completely different. Tracking tells you what’s true for you.

What helps

If PMS is severe

Severe mood changes before your period may be premenstrual dysphoric disorder (PMDD), which is treatable. Talk to your doctor.

A real-life example

Hana, 37, noticed that every month, around day 22, she felt ravenous on Wegovy. After three cycles of tracking, the pattern was clear. She now keeps Greek yogurt and edamame on hand that week, and it no longer feels like the medicine has stopped working.

Common questions

Does Wegovy stop working before my period?

It doesn’t stop working, but some women notice more hunger in the luteal phase.

Can GLP-1s make PMS worse?

There’s no clear evidence. Track symptoms and tell your doctor.

Should I change my dose around my period?

No. Never change your dose on your own. Talk to your prescriber.
